There has been a lot of speculation about who or what is the “Something to Fear”. Based on the titles of the other chapters I believe Rick and co are the something to fear. The titles tend to describe Rick and his group, as shown below:

Days Gone By – A small bit of life before the ZA for Rick and Shane (days gone by as in nostalgia) and Rick’s introduction to what happened when he was in a coma (days gone by as in missed). Shane dies signalling the end of Carl’s childhood and Rick’s life as he knew it (days gone by as in never to return)
Miles Behind Us – They continue to get farther away from what they knew: their life before the ZA, Shane, the quarry, the farm... Safety Behind Bars – The survivors are safe from zombies in the prison, though not safe.
The Heart’s Desire – The love triangle, the baby, the Glenn-Maggie romance...
The Best Defense – I got nothing for this one, unless it is meant ironically. The best defence is a good offense, but this arc has the abuse of Michonne, the loss of Rick’s right hand, Lori worrying hubby’s dead and not having a boy toy to take the pain away. This is the lowest point so far for many of them. I guess Rick, Michonne, and Glenn did go on the offensive with their riot gear, but they ended up bringing more trouble than they ever thought possible.
This Sorrowful Life – I have trouble with this one too. It makes more sense for the preceeding arc. All I can think is that 5 through 7 is really one long arc, not three separate ones.
The Calm Before – The survivor’s experience relative calm, fortifying the prison.
Made to Suffer – They are made to suffer by the Governor.
Here We Remain – Rick and Carl are all that remains (or so they think)
What We Become – Rick and Abraham discuss the type of men they were before, and what they have become in the new world.
Fear the Hunters – They fear the hunters, the hunters definitely don’t fear them (until it’s too late).
Life Among Them – Them being the seemingly peaceful settlement of Alexandria, and the survivors’ potential for life among them
Too Far Gone – Carl in particular is too far gone to play with other kids. Rick and Michonne fear they may be past reintegrating into society.
No Way Out – Literally. They are surrounded by zombies and trapped in Alexandria.
We Find Ourselves – This is where Rick realizes the zombies are not so much of a threat, if they work together. In a way they find themselves like collage students find themselves, establishing their identity in this new world; they are no longer lost. More literally they find themselves strong and capable.
A Larger World – The survivors are introduced to a larger world and Rick starts to see potential.
(I apologize, it has been a while since I read the first few volumes so I may be a little off, but the main points are there)
All the titles refer to the survivors and their situation, which would mean they ARE Something to Fear, not HAVE Something to Fear.

Interesting theory, in that the Volume names are always descriptive of the group's current state (with the exception on A Larger World). Although, We Find Ourselves and A Larger World together make a statement of the group's state...
So perhaps Rick and co are Something to Fear. Its kind of a bait and switch that Kirkman pulled, with having the Saviors featured on the covers, causing us to assume that they are something to fear.

The recent ones especially the larger world shows rick parniod of a new group and says if we don't like what we see we kill them and take what they have(something like that), quite a different statement then the past rick, plus the fact he kills a man and acts as if it was nothing, but I don't think it will be a over night switch it going to be a build up maybe the next 100 issues will be him trying to be a good leader and turning into what he hates, fears and becoming a leader much like the governor.. hes already made similar decisions/actions and the way they drew rick in the larger world his face was almost always in shadows.
After Rick's statements about the Hilltop & other communities if they don't like them or they are a threat they'll essentially destroy them and take them over, plus taking out 7 of Saviors guys so far, it's logical to assume Rick & Co are "Something to Fear," but remember Rick said that Michonne saw "Something to Fear" in the other communities, specifically the Saviors. We'll see. Where is the twist, Kirkman?
Could you imagine the look on Rick's face if they overtake the Saviour camp and while they are cleaning it up he flips over a body to see a newborn baby crushed under its murdered mother. I think that would be a pretty dark place for Rick.

I could definitely see this and I've suspected it. There's just something not right about Hilltop.

This is what i think might happen something similar to what you said, maybe in 99 we see hilltop destroyed by saviors and rick goes out for revenge only to find out they killed families and children.. just theories though.
