I'm pretty much with you, Dead Gov... I'm thinking 149 will be Negan vowing to do a dirty deed for Rick (sort out Alpha and the Whisperers in some way) in exchange for his freedom and his return to the Sanctuary. Instead, Rick just straight up murders him in public to show everyone that he's done playing nice guy with the rules.
Alternatively, you could also just call me Soiled_Panties, on account of how pissy I can be.

I like how ominous the cover looks, but I could do without the giant 150 in the background. I honestly think it's still a little premature for Rick to die at 150. I am more inclined to think he'll go out in issue 200.
As for the betrayal aspect I think it would be an interesting scenario for Rick to be forced by Alpha to choose between saving Carl's life or saving ASZ. Alpha was put in a similar circumstance earlier when she had to decide to send Lydia away in an effort to both protect her daughter and preserve Alpha's role as the leader. She may believe Rick is partially to blame for that situation developing and could want retaliation depending how the next few issues unfold.

Another possibility: Eugene, Carl and Lydia team with some from Hilltop against Rick's orders... Some of them are killed, possibly Carl. Eugene returns, feeling defeated, and goes to Rick for forgiveness. Rick goes batshit on him, resulting in the bloodiest murder we've seen yet.

150-RICK Is betrayed by.... LYDIA, and ANDREA is dead, blood on Rick could either be Lydia's or Andreas..
MY REASONING:
147-On cover, Andrea, Carl , & Lydia are traveling at night, I'm Sensing Lydia actually DOES get The Whisperers brought on Andrea & Carl & herself, either purposely or inadvertently. This results in them being taken.
148-Lydia is back to her whisperer home, back in a skinsuit, and On cover is he crying; I'm guessing she's now caused the death of 1 of her "Guests". (My Money's definitely on Andrea more than Carl)
149-Rick is freaking out, Andrea+Carl are MIA, as well as Lydia. He knows something is wrong, (especially given his appearance on cover)-Therefore,desperate times call for desperate measures; he frees Negan.
150-Given the tagline"betrayed",I'm guessing in 147 he may have sent Lydia to aide Andrea / Carl on some sort of Recon mission, or something like that, yet Lydia either sets them up or accidentally gets them caught, resulting in death of 1 of his loved ones... THEREFORE, WITH Negan acting as the devil on Rick's Shoulder, the Pacifist Rick Monroe side of Rick Dies, and BrutAl Old School take-no-shit Rick comes back for 1 last hoorah... I'm guessing his death will follow shortly afterwards, (story wise), that way when it finally happens, we remember Rick forever as the badass he truly is deep-down, regardless of how "civil" he pretended to be lately.
For some reason, after seeing this cover and the few prior, I'm dead set that something similar happens...

I think either the Saviors, Kingdom, a band of angry people, or a combination of all of those decide to go against Rick's wishes and attack the Whisperers. What ensues is a brutal massacre where the Whisperers destroy everyone that attacked them.
It makes sense that it could happen since it is issue 150 and something big happens at these landmark issues. Also, because it would drastically cut down on many of the characters in a single Whisperer horde attack. Just imagine...only Hilltop and Alexandria left standing.
